I give and bequeath [describe gift] to Lakeshore Community Foundation, Inc. (“Foundation”), Manitowoc, Wisconsin, to be used by it for its general charitable purposes as defined in and subject to the Foundation’s Articles of Incorporation and By-Laws, as amended from time to time.

 

I give and bequeath [describe gift] to Lakeshore Community Foundation, Inc. (“Foundation”), Manitowoc, Wisconsin, to be added to, or used to create, a separate fund known as [name of fund]. I desire that the distributable earnings, as determined by the Foundation’s Board of Directors, be used for its general charitable purposes as defined in and subject to the Foundation’s Articles of Incorporation and By-Laws, as amended from time to time. I direct that any payments from said fund be made to organizations to be determined by the Foundation. It is my desire that the fund be of indefinite duration.

 

I give and bequeath [describe gift] to Lakeshore Community Foundation, Inc. (“Foundation”), Manitowoc, Wisconsin, to be added to, or used to create, a separate fund known as [name of fund]. I desire that the distributable earnings, as determined by the Foundation’s Board of Directors, be used for the following purposes:  [state purposes]. I direct that the fund be administered according to the rules and regulations governing the Foundation, as well as its policies. I desire that any payments from said fund be made only to such qualified charitable organizations as will carry out the purposes specified above and that said organizations and the amounts paid thereto be determined by the Foundation.  It is my desire that the fund be of indefinite duration.

(“Foundation”), Manitowoc, Wisconsin, to be added to, or used to create, a separate fund known as [name of fund]. I desire that the distributable earnings, as determined by the Foundation’s Board of Directors, benefit the following qualified charitable organizations: [name of organizations]. In the event any of the above-specified organizations cease to exist, lose their tax-exempt status, or no longer qualify as a charitable organization for any reason, I authorize the Board of Directors of the Foundation to select another organization to benefit from said fund which, in its sole discretion, is similar in purpose to the originally specified organization. I direct that the fund be administered according to the rules and regulations governing the Foundation as well as its policies. It is my desire that the fund be of an indefinite duration.
